I need some help involving DHS and my son?

im needing to report my ex for child neglect because she isnt feeding him properly he is one year old and she is still feeding him formula she rarely changes him unless there pee or crap literaly coming out of his dyper she constantly has him in a car seat or she is constantly holding him she is always leaving him with people just so she can go run around town with her friends and im sick of it so how exacly would i go and report this to DHS do i just go in and say *hey i need to report my ex for child neglect* ? because i dont wana go in there looking like im just talkin **** so could someone plz help me out
